Procedure
I. Apply.
II. Attend
III. Monitor
a. proof of participation in TALES session; and
b. online reflection paper.
2. Proof of participation includes:
a. presentation of original certificates
b. pictures
c. Cultural app-generated certification
d. Other documents that may be collaboratively agreed upon by SD and the coordinating unit.
3. The collaborating unit’s director/head may also email SD the list of students who completed the TALES session as
proof of participation.
4. Secure from SD a slip indicating your completion of the TALES Session including the number of hours rendered.
5. SD may not credit your attendance to a TALES session due to late or incomplete attendance, plagiarism of reflection
paper, and other acts of dishonesty in the completion of TALES monitoring requirements.
- Register online prior to attendance in any TALES session. Go to the "Sign Up" page to register.
- Apply for TALES sessions according to the type of offense that you incurred. There are minimum requirements for each type of offense.
- Wait for Student Discipline's confirmation via email or SMS. Attendance will not be credited unless confirmation is received prior to the TALES session.
II. Attend
- Attend the TALES session you signed up for. Walk-in registration is not allowed.
- On the registration area on the day of the event, check your name on the list provided by SD to the coordinating unit or show the email of confirmation from SD.
- Prompt attendance is required. You must also finish the entire TALES session to earn credit points. Latecomers are not allowed and will be asked to register for another TALES schedule.
- Non-attendance/no-show on the TALES session you signed up for will constitute a minor offense of simple misconduct. Exemption may be given for students with meritorious reasons, subject to validation.
III. Monitor
- The student must submit to Student Discipline (SD) (located at the G/F of NRH) the following:
a. proof of participation in TALES session; and
b. online reflection paper.
